When life gets complex, we need a plan. When our health becomes complex, we need a Care Coordinator.

The Grand Bend Area Community Health Centre’s, Megan Elliott, provides care coordination when patients need help setting up health supports and resources.¬†She acts like a system navigator looking after the details to get the best care possible. Megan works with patients to understand their unique health care needs. She helps develop a plan that provides the best support while making good use of health care resources. The results are increased patient autonomy and capacity to meet individual health goals.
