Model of Health and Well-being

The model that guides our approach to comprehensive primary health care.

Grand Bend Area Community Health Centre is one of 83 Community Health Centres (CHCs) in Ontario. The Model of Health and Well-being unites all CHC’s with three principles and eight attributes which guide the delivery of comprehensive primary health care.

The World Health Organization defines health as “a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity.” Our model is structured around this belief.

Our Guiding Principles

High Quality, People and Community-Centred:
Continually striving to meet unique and specific patient needs through a community-governed approach, always taking into consideration the key determinants of health.

Health Equity and Social Justice:
We advocate for public policy that fosters a healthier community. Our programs and services are designed with health equity and social justice in mind.

Community Building and Belonging:
Nurturing a community where people feel safe and a sense of belonging is a priority.
