You are not alone, we are in this together! No truer words have been said than now during the COVID-19 pandemic. The Grand Bend Area Community Health Centre is working with community partners, public health and government ministries to ensure your health and safety are protected during this unprecedented time. We welcome all area residents to contact us for medical and mental health assistance.

This COVID-19 page contains essential links to public information and resources that you will need to come through the pandemic safe and sound. If you know of a good resource, particularily in the Grand Bend area, please email us so that we can include it here.

COVID – 19 Drive-Through Testing

 

The Health Centre offers drive-through testing and virtual assessments for COVID-19, Monday to Friday 1:30-4 pm ( except Holidays).

Please complete the self assessment to determine if you have symptoms and require testing.

In alignment with Ministry of Health direction, COVID-19 Testing at the Health Centre will be provided to those that:
• Are showing COVID-19 symptoms;
• Have been exposed to a confirmed case of the virus, as informed by their public health unit or exposure notification through the COVID-19 app;
• Are a resident or worker in a setting that has a COVID-19 outbreak, as identified and informed by their local public health unit; or
• Are eligible for testing as part of a targeted testing initiative directed by the Ministry of Health or the Ministry of Long-Term Care. Ontario Health will continue to notify Assessment Centres of any targeted testing.

Low risk, asymptomatic individuals are not being considered for COVID-19 testing to ensure those at greatest risk are tested.

A member of the COVID-19 Assessment Centre team will call you to book an appointment for a test.

We Are Open!

The Grand Bend Area CHC continues to supply primary care and Allied health services during the COVID 19 crisis.

All patients/clients entering the Centre will be screened at the front door and required to wear a mask.

It is very important for you to share all of your symptoms with us when being screened in order to determine the right route of care for you that is safe for both you and your provider.  

Care may be given virtually by phone or by computer or your provider may choose to see you in person.   

We wish to care for you and the whole community by keeping everyone safe. Help us to help you by supplying the correct information so we can address your health and wellbeing needs.
