In an integrated, coordinated and collaborative environment, we provide our patients with quality, primary health care. Our interdisciplinary team works together to assess, diagnose and manage health issues, always considering the factors that impact the health and well-being of our clients.

Our interdisciplinary team includes:

  • Family physicians
  • Nurse practitioners
  • Physician assistants
  • Registered practical nurses
  • Medical office assistants

The allied health members of our team include: registered social workers, nurses, dietitians, respiratory therapists, physical therapists and health promoters. They have expertise in mental health and chronic disease issues, rehabilitation, wellness care and health promotion. Together, we work with patients to address the barriers to their health.


Diagnostic Services

Along with health promotion, counselling, patient education and disease prevention, our centre offers diagnostic services such as:

  • A twice daily lab courier
  • Ambulatory blood pressure monitoring (ABPM)
  • Event/looper monitoring
  • Spirometry
  • Stress testing

Care Coordination

Using a collaborative approach, a Care Coordinator is available to work with you and your health care team to identify your health and well-being goals and develop a care plan.

Patients at both Grand Bend and Hensall sites can access primary health care by appointment, call 519-238-2362.
