Transitioning to a New Strategic Plan

The 2022–25 plan, Rebounding & Coming Back Strong, aimed to rebuild capacity post-pandemic and enhance team-based care and communication. Key outcomes included growth in panel size, improved community connections in underserved areas like Hensall, and increased demand for programming. The new 2025–28 plan builds on these foundations, shifting to strategic expansion and system alignment. It introduces a data-driven, equity-focused model, the Patient Medical Home framework, and specific metrics linked to best practices and health outcomes.

Quality Improvement Plan

The GBACHC has developed a Quality Improvement Plan (QIP) that aligns with its strategic plan. With the GBACHC’S core values acting as a foundation for the QIP, the plan aims to improve service effectiveness and patient/client experience in a safe and timely manner while ensuring equitable treatment for everyone.
