Transitioning to a New Strategic Plan

The Grand Bend Area Community Health Centre’s 2025–2028 Strategic Plan sets a clear direction for the next three years, focused on strengthening access to care, improving health outcomes, and responding to the changing needs of our growing rural communities. Grounded in our mission to enhance the health and well-being of the people in our communities, the plan is built around five key priorities: value for money, health outcomes, patient experience, provider experience, and equity.

The plan reflects both local realities and broader health system change. It responds to rising rates of chronic disease, mental health needs, an aging population, workforce pressures, and the growing number of people without access to primary care. Over the next three years, the GBACHC will focus on expanding access through team-based and virtual care, strengthening prevention and chronic disease management, supporting staff well-being and leadership, and improving equitable access for people facing social, economic, and health barriers. Together, these priorities position the GBACHC to continue serving as a strong, compassionate, and innovative hub for community health in the region.

Quality Improvement Plan

The Grand Bend Area Community Health Centre’s 2026–2027 Quality Improvement Plan outlines how we are strengthening access, experience, equity, and efficiency across our services. Key priorities include welcoming and attaching new patients, improving timely access through same-day and flexible in-person or virtual appointments, strengthening triage and team-based care, and using digital tools to reduce administrative burden and support smoother transitions in care. These efforts reflect our commitment to providing the right care, from the right provider, at the right time.

The plan also reinforces our focus on safe, respectful, person-centred care. The GBACHC is increasing staff education in equity, diversity, inclusion, and anti-racism, while continuing to use patient, client, and community feedback to shape service improvements, planning, and decision-making. Together, these actions help ensure our services remain welcoming, culturally safe, and responsive to the diverse communities we serve.

Quality Improvement Plans are part of a province-wide Ontario process that helps health care organizations set annual improvement priorities and track progress. The QIP program has roots in Health Quality Ontario and is now supported by Ontario Health, which provides guidance, indicators, and the QIP Navigator platform used to develop and submit annual plans. For the GBACHC, this means our local improvement work is grounded in community needs and aligns with broader quality goals across Ontario’s health system.
