Ongoing diabetes education and care to help people live a healthier life.

The GBACHC offers patient-centered diabetes education on how to self-manage diabetes and prevent or delay complications from the disease.

Our Program

The Diabetes Education Program (DEP) provides assessment, education and support to individuals living with diabetes. This includes nutrition and physical activity education and recommendations, medication education and recommendations, basal and bolus insulin starts and adjustments (with a medical directive), and foot assessments.

Self-referrals (without a physician referral) are welcome!

The DEP can see and assess people living with the following types of diabetes:

  • Prediabetes
  • Type 2 Diabetes
  • Type 1 Diabetes (with or without a pump)
  • Gestational Diabetes
  • Individuals in need of injectable therapy education

Diabetes education and cooking classes are scheduled throughout the year.
