The Grand Bend Area Community Health Centre (GBACHC) seeks a full-time Registered Practical Nurse (RPN) to join our interprofessional primary care team. The GBACHC provides comprehensive, team-based care to clients and families across Grand Bend, Hensall, and surrounding communities, with a strong focus on health promotion, chronic disease management, illness prevention, and equity of access.

Role Overview

The RPN works as an integral member of the interprofessional care team, collaborating with physicians, nurse practitioners, registered nurses, social workers, dietitians, health promoters, and other allied health staff. Key responsibilities include the following:

  • Providing direct nursing care, including health assessments, injections, immunizations, wound care, medication administration, and chronic disease support
  • Supporting health education and self-management strategies for individuals and groups
  • Participating in care planning, case conferencing, and quality improvement initiatives
  • Promoting preventive care and screening (cancer screening, immunizations, chronic disease monitoring)
  • Maintaining accurate and timely documentation in the electronic medical record (EMR) as per CNO and GBACHC standards

Qualifications

  • Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse
  • Diploma in Practical Nursing from a recognized institution
  • Current certification in CPR (Level C) and AED
  • Experience in primary care, community health, or chronic disease management (preferred)
  • Demonstrated commitment to interprofessional, client-centred care
  • Strong communication, organizational, and critical-thinking skills
  • Proficiency with electronic medical records (EMR) and computer applications
  • A completed Vulnerable Sector Police Check completed within the last six months
