Ongoing diabetes education and care to help people live a healthier life.

The GBACHC offers patient-centered diabetes education on how to self-manage diabetes and prevent or delay complications from the disease.

Our Program

Our program includes problem solving, goal setting and active participation in decision-making. People with diabetes will be supported in interpreting and acting on:

  • Lifestyle issues (i.e. foot care)
  • Making informed management decisions about insulin
  • Medication
  • Nutrition
  • Physical activity
  • Self-monitoring of blood glucose results

Diabetes Education

Attending the Diabetes Education Program

Any adult, 18 years and older, diagnosed with pre-diabetes or at risk of diabetes, Type 1 or Type 2 diabetes is able to access this program at our Centre.

Bringing the Community Together

Community Diabetes Programing is offered depending on the current needs of the communities we serve.
