In an integrated, coordinated and collaborative environment, the GBACHC provides patients/clients with quality primary health care. An interdisciplinary team works together to assess, diagnose and manage health issues, always considering the factors that impact the health and wellbeing of the patient/client.

Interprofessional team members include: registered social workers, nurses, dietitians, respiratory therapists, physical therapists and health promoters. They have expertise in mental health and chronic disease issues, rehabilitation, wellness care and health promotion. Together, they work with patients/clients to address health barriers and therapies.

Diagnostic Services
Along with health promotion, counselling, patient education and disease prevention, the GBACHC offers the following diagnostic services:
- Twice-daily lab courier
- Ambulatory blood pressure monitoring (ABPM)
- Event/looper monitoring
- Spirometry
- Stress testing

Care Coordination
Using a collaborative approach, a Care Coordinator is available to work with patients/clients who have complex health issues to identify health and well-being goals and to develop a care plan.

Find a Family Doctor or Nurse Practitioner
Our primary care patient intake has transitioned to Health Care Connect. It refers Ontarians who don’t have a physician to a family health care provider who may be accepting new patients.
Register yourself and/or your whole family for Health Care Connect, and a care connector will search for a doctor or nurse practitioner who is accepting new patients in the community.
