When life gets complex, we need a plan. When our health becomes complex, we need a Care Coordinator.

The Grand Bend Area Community Health Centre’s, Megan Elliott, provides care coordination when patients need help setting up health supports and resources. She acts like a system navigator looking after the details to get the best care possible. Megan works with patients to understand their unique health care needs. She helps develop a plan that provides the best support while making good use of health care resources. The results are increased patient autonomy and capacity to meet individual health goals.

Megan EMegan is a Registered Practical Nurse who understands the services within the Grand Bend Area Community Health Centre. She also works with partners in the community, including the Huron Perth Health Link, Lambton County Lake Huron HealthLink, Home & Community Care, and dozens of other community support agencies.

Each patient Megan works with receives:

  • An assessment to determine needs.
  • An individualized service/care plan.
  • Case conferencing to engage supports.
  • Referrals to the most appropriate supports.

Coordinated care planning is an important part in providing a holistic, inter-professional approach to all aspects of a patient’s health and well-being.
