The Grand Bend Area Community Health Centre (GBACHC) is pleased to have received provincial funding to increase our physician and nurse practitioner staffing, allowing us to expand our patient roster.
The GBACHC’s commitment to the funder, Ontario Health, is to add up to 1600 patients over the next 12 months. Ontario Health directs us to initially focus on unattached patients on the Health Care Connect list.
If you wish to become a patient at the GBACHC, it is recommended that you register through the provincial patient attachment portal, Health Care Connect. Since November 2025, the GBACHC has contacted and rostered over 300 patients. Based on this experience, registrants can expect us to contact them within three months of their application being identified on the Health Care Connect list. If you have an urgent health issue, contact us or visit our Saturday morning walk-in clinic for assistance.
