The Grand Bend Area Community Health Centre (GBACHC) is seeking a 1.0 full-time family physician to join our dynamic, interprofessional primary care team. Located along Ontario’s beautiful Bluewater coast, the GBACHC provides comprehensive, team-based care to diverse populations across Grand Bend, Hensall, and surrounding communities. Role Overview

As a family physician at the GBACHC, you will work in a collaborative model alongside nurse practitioners, registered practical nurses, dietitians, social workers, physiotherapists, occupational therapists, health promoters, and other inter-health professionals. Physicians at the GBACHC play a key role in:

  • Providing comprehensive, evidence-based and holistic primary healthcare, following the Health Disciplines Act of Ontario, the CPSO practice standards, and Community Health Centre (CHC) guidelines.
  • Supporting chronic disease management, preventative care, and health promotion initiatives.
  • Participating in interprofessional case conferencing and quality improvement initiatives.
  • Contributing to the teaching and mentorship of learners when applicable.

Qualifications

  • Independent practice license with the College of Physicians and Surgeons of Ontario (CPSO).
  • Certification in Family Medicine (CCFP) from the College of Family Physicians of Canada (CFPC) or equivalent experience is an asset.
  • Strong commitment to interprofessional, team-based primary care.
  • Experience and/or interest in community health, chronic disease management, and preventative care.
  • Demonstrated commitment to equity, diversity, and inclusion in healthcare.
  • A completed Vulnerable Sector Police Check completed within the last six months.
