The Grand Bend Area Community Health Centre (GBACHC) is seeking a compassionate, collaborative, and community-focused Social Worker to join our interdisciplinary primary care team on a full-time basis.

Position Summary

As part of a multidisciplinary team, the Social Worker provides clinical intervention, assessment, supportive counselling, and community resource navigation to patients and families within the GBACHC catchment area. The successful candidate will work collaboratively with allied health professionals, primary care providers, and community organizations to support the mental health and overall well-being of our patients.

  • Location: Grand Bend and Hensall locations
  • Salary Range: $77,100 – $96,400 annually

Qualifications

The successful candidate will possess:

  • Bachelor of Social Work (BSW)
  • Master of Social Work (MSW) or equivalent MA preferred
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers
  • Advanced counselling and intervention skills
  • Strong assessment and documentation abilities
  • Knowledge of program planning, development, and implementation
  • Excellent communication and interpersonal skills
  • Demonstrated knowledge of community resources and supports
  • Strong computer skills, including proficiency with Electronic Medical Records (EMR) systems

Key Responsibilities

Clinical & Professional Practice

  • Provide individual and family counselling and therapeutic interventions
  • Complete comprehensive psychosocial assessments and develop care plans
  • Maintain professional practice standards in accordance with the Ontario College of Social Workers and Social Service Workers
  • Foster collaborative and therapeutic relationships with patients, families, and team members
  • Maintain confidentiality and uphold ethical standards of practice
  • Participate in ongoing professional development and evidence-informed practice

Team & Community Engagement

  • Collaborate within an interdisciplinary healthcare team
  • Support the development and implementation of community-based programs
  • Participate in community partnerships, professional organizations, and outreach initiatives
  • Contribute to student supervision, research opportunities, and grant development as applicable

Health & Safety

  • Support a safe and respectful workplace environment
  • Adhere to Occupational Health and Safety policies and procedures
  • Participate in organizational initiatives that promote staff and patient well-being

Working Conditions

This role is based in a standard office and clinical environment and may require flexibility in scheduling to meet organizational and patient needs.
