The Grand Bend Area Community Health Centre (GBACHC) is seeking a compassionate, collaborative, and motivated locum family physician to join our interdisciplinary primary care team on a full-time basis.

Position Summary
Working with the interdisciplinary team, the locum family physician will assist with health promotion, illness prevention and treatment, health education, awareness, counselling and education. The physician will also provide a full range of primary health care interventions.

- Hourly Rate: $170.24 per hour
Qualifications
- Current registration and in good standing as a physician with the College of Physicians and Surgeons of Ontario (CPSO)
- Primary care, community health, hospital, or public health experience; primary care experience is an asset
- Knowledge and experience related to mental health, addictions, and chronic disease management are considered assets
- Strong clinical judgment, organization, communication, and ability to work independently and collaboratively within an interdisciplinary team
- Strong computer skills and experience with electronic medical records; PS Suite EMR experience and familiarity with local resources are assets
Key Responsibilities
Clinical & Professional Practice
- Manage patient same-day and booked visits
- Work collaboratively with the health promotion team in the design and implementation of services and programs to address the social determinants of health
- Responsible and accountable for the comprehensive assessment, diagnosis, treatment, health education and counselling, and other clinical activities according to CPSO standards of practice for physicians
- Provide harm reduction strategies that support a balanced state of well-being among patients
- Participate in case conferences with other providers to ensure coordination of services to the patient
- Maintain a level of care that meets wholistic and ethical standards based on CPSO requirements
- Provide most responsible provider cross coverage within the primary care team
- Strictly adhere to the confidentiality and privacy policies and all other policies and procedures of the GBACHC
- Ensure that effective and efficient use is made of all resources available to the GBACHC to respond to the health and wellness needs of the community, including internal referrals to other staff and programs as appropriate
- Perform other related duties and responsibilities consistent with the scope and intent of the position

Working Conditions
