We’re building the future of healthcare in Grand Bend, Hensall and the surrounding area – and you’re invited to help roadmap it.
Join the Grand Bend Area Community Health Centre (GBACHC) leadership team and Board on April 14 from 3:00–5:00 pm in the Community Room, 69 Main St. E., Grand Bend for our Strategic Plan Open House. This is your chance to learn more about how we’re working to better serve our community – and how your feedback can make a difference.
Our draft strategic plan is rooted in the Quintuple Aim: improving health outcomes, patient and provider experiences, health equity, and value for money. With a clear focus on mental health, chronic disease management, addiction care, and access for unattached patients, we’re designing care that is compassionate, connected and community-led.
Whether it’s through new virtual care options, partnerships with public health and local hospitals, or expanded outreach to vulnerable and racialized populations, the GBACHC is committed to delivering the right care at the right time.
The Open House is more than just an information session – it’s a conversation. Come share your thoughts, meet our team, and see how we plan to turn community needs into action. Your input will help us ensure that programs and services reflect the real lives of those we serve.
